NEW STUDENTS

​Required Documents

​

1. Two proofs of residency (all grades). Any of the following is acceptable proof of residency: a deed, a lease, current utility bill, current credit card bill, property tax bill, vehicle registration, driver's license, or a government issued ID. ​​​

2. A Completed child's physical and immunization form.

​3. ​A child Dentist report

4. A copy of the child's birth certificate in English.

5. If the child is 4-5 years of age: ​ Kindergarten Teacher Recommendation, 

6. If the child is 6 years of age or older:  Teacher Recommendation filled out by the child’s previous teacher.

7. If the child is 6 years of age or older: Transcript Request form filled out by you and mailed to your child’s previous school.​

Please be advised that:
1- Only applications with documents marked as satisfactory/fulfilled are processed and evaluated.
2- For Physical and dental forms: some medical offices may take longer to fill the required forms. Kindly upload a note stating the approximate time of receiving and uploading these forms.  This is only valid for physical and dental forms.
